STEP 2: Send Info

Send us an image of your drivers license and the front and back of your insurance card to clinic@rootwholebody.com

Once received, we will confirm your network status within 2 business days.

STEP 3: Claim Submission

Root will bill your insurance plan. You will be responsible for any unpaid balance.
