STEP 1: Self-Verify
please confirm your benefits cover your preferred clinician by visiting your insurance plan website (links below):

STEP 2: Send Info
Send us an image of your drivers license and the front and back of your insurance card to email@example.com
Once received, we will confirm your network status within 2 business days.
STEP 3: Claim Submission
Root will bill your insurance plan. You will be responsible for any unpaid balance.
